Development of an integrated system for hospital functional assessment: Linkage between codes from the Brazilian classification of physiotherapeutic diagnoses, mobility and the international classification of functionality and mobility scales to propose quality indicators and therapeutic plans

Introduction: Functional assessment is a fundamental pillar of hospital clinical practice, especially in complex settings such as intensive care units, where patient mobility is a critical indicator of recovery and prognosis. In the Brazilian context, the integration of mobility scales with standardized classification systems, such as the Brazilian Classification of Physiotherapy Diagnoses (CBDF) and the International Classification of Functioning, Disability, and Health (ICF), represents a significant advance in improving the quality and standardization of care. Objective: The linkage between hospital mobility scales and codes from the Brazilian Classification of Physiotherapy Diagnoses and Mobility and the International Classification of Functioning was systematically compared, developing indicators of care quality and integrated treatment plans to guide practical implementation in a Brazilian hospital setting. Methods: This comparative methodological study followed established guidelines for correspondence between assessment instruments and functional classification systems. The Intensive Care Unit Mobility Scale and the Johns Hopkins Highest Level of Mobility Scale were used. The process followed a systematic methodology of parallel conceptual analysis and expert consensus in structured stages. Descriptive statistics, Fleiss's kappa coefficient, and multiple correspondence analysis were applied. Results: Conceptual overlap between systems reached 78.6%. Twenty-one quality indicators were developed, and the integrated model demonstrated superior performance across all indicators, with particular emphasis on safety and institutional quality. Conclusion: The integrated hospital functional assessment system allows for the generation of highly clinically relevant quantitative and qualitative information, contributing to greater transparency in care processes, strengthening physical therapy auditing, improving care management, and supporting evidence-based clinical decision-making.
