Adherence to physical exercise after bariatric, surgery determinant factors and Impacts on body composition and quality of life: a systematic review

Introduction: Bariatric surgery is effective for the treatment of morbid obesity. However, adherence to physical exercise in the postoperative period is necessary to maintain the benefits achieved. Objective: To identify the determining factors of adherence to physical exercise after bariatric surgery and to evaluate its effects on body composition and quality of life. Methods: Literature review, registered in the Open Science Framework (OSF) DOI: 10.17605/OSF.IO/9H6NU. The bibliographic search was conducted in the PubMed, Scielo, and BVS databases between October 2, 2024, and April 31, 2025. The following descriptors were used: “obesity,” “bariatric surgery,” “physical activity,” “quality of life,” “body composition,” and “adherence,” combined with Boolean operators (AND, OR). Studies published between 2000 and 2024, in Portuguese, English, and Spanish, addressing outcomes in patients up to 60 months after surgery were included. Case reports, letters to the editor, conference abstracts, and articles in unspecified languages were excluded. Results: Adherence to physical exercise is associated with lifestyle changes, including regular physical activity, healthy eating, and multiprofessional follow-up. Mental health plays an important role in maintaining these habits. Quality of life tends to improve after surgery, especially when healthy behaviors are adopted. Conclusion: Adherence to physical exercise is not directly associated with improvement in quality of life, suggesting the influence of multiple factors. Barriers such as physical limitations, external factors, and motivational aspects should be considered. More specific investigations are needed to identify the determinants of adherence and fill existing gaps in the literature.
